 John "Jack" Collins was killed on May 13th 1944 in Italy
while serving in the United States Army, TEC. 5-350th Infantry.
He was in position as a forward observer for artillery when
killed in action.

Jack was born on May 16th 1921 in Beacon NY
His mother died when Jack was an very young and
was raised by his father John Collins Sr. along with
his brother Steve "Buddy" Collins. His cousins
are James, Frank and Ralph Collins.
